In Bollywood scenes during intimate scenes, they just act or they might have some real thing?

Answer:-

In Bollywood, the intimate scenes are done with a lot of suave and class. The "nude" scenes are very rare but present nonetheless. Two actors might get turned on because of the risque scene but they definitely don't do it on camera.
Considering a scenario where the co-actors are real life couples as well, the level of comfort must be really high with less awkward moments between them.
A funny anecdote from Titanic goes that Kate Winslet stood naked infront of Leonardo di Caprio on the first day of shoot to help him get acquainted with her naked presence around him.

If the co-actors are not in a relationship, I'd imagine that it must be really awkward to be sexually comfortable with them. They are ordinary human beings and it is only logical for them to be a little weird about doing it with strangers.

To start from the basic
An intimate scene is usually shot in close ups or even extreme close up of skin and lips and body.
Now imagine a camera - Google search- Arri Alexa xt or red epic (currently movies are being shot on) Imagine this camera at a distance of 3-4 feet away from the actor. Behind that camera is the cinematographer, focus puller the camera attendant and a couple assitants. The director and the script supervisor are in the room to.
Usually in Bollywood intimate scenes are shot with limited crew around. So all this is happening and the shooting lights are all around you. In this kind of an atmosphere there is a thin chance of anything happening. Also the pressure of the director expecting a particular way of intimacy to be displayed on screen is completely out of this world. You never know how your fingers, lips and other body parts look onscreen while making love, so its sometimes difficult to express certain emotions ( unless youve been shooting yourself alot ;) ;))
Also after the few minutes of the shot the director says cut and hell breaks loose on set, with the hair stylist coming in to touch up the actors hair and the costume guys working on their stuff and so on.  So its v difficult I would say to even perform such scenes , leave aside something happeningI

Imagine yourself, partially nude and surrounded by 20 to 50 people. Every one watching you, director shouting at you, asking to pose from this side, turn here, look there, hold it and then CUT IT.  Take a break of 15 minutes and start again.

Lightmen showing lights on your face. For water (rain) or bath scene a group of people showering water over you. You need to act in cold or warm water, sometime water will enter in eyes. You or other partner loose focus. Again breaks in between.

And one initmate scene takes normally 4 to 6 hours to shot! So imagine, how easy is to feel. In fact you feel fully tired, no chance of other kind of feel at all!

Sex, as an act, is more of psychology, than physiology. When your mind is unwilling and not ready, when your mind is conditioned and when are under compulsion, sex cannot happen unless your extremely repressed feelings are being unleashed.
While enacting intimate scenes the stars are conditioned and constrained to be within the limits of normal professional interaction. No person can stop the natural flow of bodily fluids while having intimate contacts but to think that they all result in sex would be wrong.
After the pack-up what happens and how they deal with each other is none of our business. For the screen they act with great chemistry and we all seem to enjoy them doing that.
